Under the direction of the Director of Care Coordination and Patient Transitions, the Care Coordinator II is responsible for coordination and management of the hospital based population. As a leader of the multidisciplinary care management team, the Care Coordinator II is accountable and responsible for coordinating care and service of a selected patient population, promo
